ACBIS Certification Forms:
CBIS Application Form Group Number is CTAT10. Please download this form, answer all questions, save it and then submit, along with your resume, and employment
verification, to your Group Administrator (Rena Thompson).
CBIS Employment Verification Form Print and have signed by your supervisor, if you do not have a
supervisor, have it signed by a Board Member or other type of Director.
